In compliance with new federal and state laws, we are required to charge sales tax for certain states.
Many states (but not all) offer Sales Tax Exemption for churches and non-profits.

  • How to Apply for Sales Tax Exemption

    Step 1

    Does my state allow sales tax exemptions? Is my organization exempt from sales tax?

    We have created the list above for your reference, but each state has different rules that may change. For any questions on if your church or organization qualifies, please contact your local state authority.

    Step 2

    Download the state specific exemption form by clicking the download icon next to your state.

    A Tax Exemption "Form" is different than your Tax Exemption "Certificate." Some states require us to collect both to approve the exemption.
    Note: Florida, Illinois and Maryland only require the certificate and no separate forms.

    Step 3

    Fill out the form via PDF editor or print to fill out by hand. Read all instructions carefully.

    The information below may be required when completing the form. Note that we are unable to accept incomplete forms.

    Select "Blanket Certificate"

    Always choose "Blanket" certificate if allowed. This ensures we can use this same form for future orders.
    Note: Colorado does not offer a "Blanket" exemption and requires a separate form for each exempt transaction.

    Type of Materials Being Purchased

    Church supplies or religious supplies will normally suffice for most forms.

    Step 4

    Print, sign, and date the completed form.

    Step 5

    Scan and email the form to tax@concordiasupply.com .

    Please include a copy of your Tax Exemption Certificate if required by your state.
    Email Title: Tax Exemption Request
    Email Message: Please include any additional information that may help us expedite your approval. Leave accurate contact information in case we need to contact you (Name, Phone, Email, Account #). If relevant, include your Sales Order # so we can immediately refund the sales tax.

    (You may also mail the form to the address listed above. Include a letter with the same information requested above.)

    Step 6

    Finished! Your documents will now be reviewed by our dedicated staff soon..

    Once approved, you will receive a confirmation email and a full refund for any sales tax on you previous order. Our team will contact you if additional information is needed.
